It won't let me enter dependants or DOB or save just the 1st screen. Have tried uninstalling & reinstalling but same result. Wish it was available for laptop so I could even check if it's just my phone. But will have to just download and post form. Not happy as dentist said it was st lukes issue on their machine and made me pay in full.

Just completed a claim on behalf of my wife and where the process asks "who is the service for?" , it doesn't give you an option. It only has one option..".For me" Hopefully, St, Lukes Health technicians will fix this little bug soon. Otherwise, a very neat app. P.S. Should have this app for Windows 8 Desktop or Laptop.
