Aultman Home Care and Hospice

Aultman Hospice and Palliative care, Home care and DME app provides a
convenient way for patients and families to access information about
services and provides an opportunity to request additional information.
For Health Care providers, it provides information about services and
serves as an easy way to make a referral. The intake department specific
to each service line will follow-up on all referrals received through the
app. All patient information received through the referral form will be
managed with HIPAA compliance.
