Most experts now have guidelines recommending survivorship plans, but no one pays to get them done. It took me about 4 hours to create the plan. Granted, I am proficient in healthcare and had digital access to most of the records. The plan would have been much better if you allowed free text into the doctor contact section. You only need symptoms with positive responses. Allow removing unnecessary follow up tests like mammograms for men. A prognosis and advanced directive tab would be welcome.
