Mit Forløb

Mit forløb - en livline til dig, der er patient i Region Syddanmark.

I Mit forløb kan du finde en række patientforløb og der vil løbende komme flere til. Under de enkelte forløb er der mulighed for at læse pjecer eller se video'er, som kan hjælpe dig som patient i det aktuelle forløb. Hvis du er pårørende kan det også med fordel læse og se materialet.

Du kan finde kontaktinformation til de afdelinger, som hjælper dig med lige dit forløb.

På nogle forløb tilbyder sygehuset, at du kan skrive sammen med personalet og indsende billeder, hvis du har brug for det.

I nogle forløb kan det være relevant, at du løbende registrerer målinger som eks. din vægt, temperatur eller blodtryk, så det er tilgængeligt for sygehuset næste gang du skal til konsultation.

Samtykke

Formålet med Mit Forløb er at give dig information, der er relevant for netop dit forløb på sygehuset samt sætte dig i stand til at kommunikere elektronisk med sygehuset.

For at du kan anvende app’en, skal du give dit samtykke til, at data fra din patientjournal. Fx aftaletidspunkter kan videresendes til dig via app’en. Dit samtykke registres i Mit Forløb og er tilgængeligt via din patientjournal.. Du skal dog være opmærksom på, at tiderne kun opdateres én gang i døgnet, så tiden kan være ændret uden du kan se det i Mit Forløb.

Hvis du registrerer målinger som eks. vægt og blodtryk kan du kun forvente, at det bliver set af en kliniker i forbindelse med en konsultation. Hvis du gemmer noget i dagbogen og noter, er det ikke tilgængelige for sygehuset.

På nogle forløb har du mulighed for at sende meddelelser til sygehuset, men bliver du akut syg skal du kontakte afdelingen eller ringe 112.

App’en er sikret med kryptering og adgangskode. Hvis du ikke har kode på telefonen bør du ikke lade app’en huske dig. Regionen er ikke ansvarlig for at data bliver tilgængelige som følge heraf. My course - a lifeline for those who are patient in Southern Denmark.

In My course you can find a variety of patient care and will continue to follow to. During each course, there is the opportunity to read brochures or watch video's that can help you as a patient in the current recession. If you are a caregiver may also want to read and see the material.

You can find the contact information for the departments that help you right your course.

Some courses offer the hospital, you can write with the staff and submitting pictures, if you need it.

In some processes, it may be relevant that you continuously record measurements as eg. Your weight, temperature or blood pressure, so it is available to the hospital next time you need for consultation.

Consent

The purpose of the My Course is to give you information that is relevant to your particular course at the hospital as well as enable you to communicate electronically with the hospital.
                                                               
For you to use the app, you consent to the data from your patient. Eg agreement times can be forwarded to you via the app. Your consent registries in My course and is available through your medical records .. You should be aware that times only updated once a day, so the time may be changed without you see it in my sequence.
                                                               
If you record measurements as eg. Weight and blood pressure, you can only expect that it will be seen by a clinician in connection with a consultation. If you save something in the diary and notes, is not available to the hospital.
                                                               
On some courses you have the opportunity to send messages to the hospital, but you are acutely ill should contact the department or call the 112th
                                                               
The app is secured by encryption and password. If you do not have code on the phone you should not let the app remember you. The region is not responsible for the data become available as a result.
