薬剤師（やくざいし、英: Phamacist, Chemist）とは、調剤、医薬品の供給、その他薬事衛生を司る医療従事者。近代的な医療制度は、医療を施す医師とは別に、医薬品を扱うために薬理学を習得した薬剤師を資格制度の上で配置し、こうした分業制度を持っている。

日本で薬剤師になるには、学校教育法に基づく大学において薬学の正規の課程を修めて卒業し、薬剤師国家試験に合格しなければならない。その後薬剤師名簿に登録申請し厚生労働大臣より薬剤師の免許が与えられる（12条）。薬学の正規の課程は、2005年以前に入学した者は4年制、2006年以降に入学した者は6年制である。

我が国では医師に薬における権限が集中しすぎており諸外国と比較して日本の薬剤師は諸権限がない場合が多く、戦後徐々に諸外国並みの権限を持つようになってきているが薬剤師後進国と言える。ただし、一部の病院・診療所では医師が診察、診断し薬剤師が処方を設計しそれを提案するという「薬物療法の担い手」として活躍している。医師は一般的に自分の専門とする科の薬物には詳しいが、他科の薬まで把握するには時間も労力も必要とするため、薬剤師に専門家としての意見を求める医師も増えてきた。また、薬剤師ならではの薬力学的観点での医師への薬物療法の提案や、相互作用については医師には無い薬剤師ならではの知識である。薬物の体内での薬物相互作用や、医薬品の混合の際の化学変化についての予測や対応は、有機化学や物理化学の知識に長けている薬剤師の力が発揮される場面である。特に抗がん剤、抗生物質、精神科薬の分野では薬剤師が薬学的知識を生かして積極的に薬物治療にかかわっている。
